A gynecologist discusses intimate hygiene and preventive measures for women over 35. What's important to know?

Generally, I recommend cleaning the external genitalia (vulva) once a day. Internal cleaning or douching is not necessary, as the vagina is a self-cleaning organ. Too frequent or too aggressive cleaning, such as with regular soap, can damage the naturally acidic protective layer of the intimate area. Here, please remember to clean from front to back and, if using washcloths, wash them at 60 degrees after each use.

What should be considered during one's period?

During the period, the cleaning routine generally remains unchanged. If you feel the need to clean the intimate area twice a day, you can do so. I recommend one thorough cleaning and one rinse with just water. More important is to regularly change pads and other hygiene products.
Intimate deodorants or similar products are not needed, as they either irritate too much or only work temporarily.

Which preventive measures should every woman over 35 have checked by her gynecologist?


Ideally, there is a relationship of trust between the patient and the treating physician, who may be aware of pre-existing conditions, cycle protocols, etc. A palpation examination including breasts and lymph nodes is standard and should be performed regularly. I recommend this twice a year, accompanied by regular self-examination at home.
From the age of 50, a digital rectal examination is integrated into the palpation. Also standard are a PAP smear between 25 and 34 years, and from 35 years, a combined examination with a PAP smear and HPV test (Human Papillomavirus) every three years.
Those with a notable family history can consult their gynecologist and, if necessary, be included in a special, more intensive screening program. There are now many optional services (IGEL-Leistungen) that can be very useful, such as a mammogram MRI.

As a mother of 5 children, I know that childbirth demands a lot from us women, both emotionally and physically. And these massive changes affect many areas, including sex life. A previously unknown burning sensation is frequently reported in my practice. This is because almost all women experience hormonally induced very dry vaginas after childbirth and during breastfeeding – even if few openly talk about it. Vaginal creams, which are available in large numbers at the pharmacy, quickly provide relief until the normal state returns. Later, towards menopause, the vagina also becomes drier again, and these lubricating creams are also very useful here. Break the taboo – almost every woman suffers from this problem from time to time.
